Behavioral Health

Rates of anxiety, depression, addiction and suicidal ideation are at unprecedented levels nationally. Further, the incarcerated are a disproportionately trauma-inflicted population. Suicide is the third leading cause of death in prisons and the leading cause of death in jails, four times as high as the general population. YesCare recognizes that our correctional institutions have become the de facto behavioral health safety net, which is reflected in our mission and approach to care.

 

YesCare offers comprehensive behavioral health services based on early and thorough screening and assessment of distress, disorder and risk. Prevention, treatment, health education, continuity of care and community reintegration services are central to our approach. We customize our behavioral health services to the needs of each patient, with an eye toward better outcomes at the individual and community level.

Medication Assisted Treatment (MAT) 

  • YesCare has accepted the challenge to assist our incarcerated patients through the opioid epidemic by building interactive and effective MAT programming. https://www.youtube.com/watch?v=IoLzmEjIH68
    Evidence-based practices guide the screening, diagnosing and treatment of substance use disorders. Innovative process improvement methodologies keep YesCare at the forefront of evolving substance use disorder and MAT best practices.
     

  • MAT is “the use of medications, in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders”. (Substance Abuse and Mental Health Services Administration (SAMHSA).

  • Per Bureau of Justice Statistics, a survey noted in 2009 that 58% of state prisoners and 63% jail detainees met criteria for drug dependence or abuse. This number has increased significantly, as an example in many of our jail contracts, the percentage average 75-80%. 

  • Drug overdoses have killed almost 1M people since 1999. In January 2021, drug overdose deaths exceeded homicides by 306.7%

  • Opioids are a factor in 7 out of every 10 overdose deaths. 

    • YesCare has committed to substance use disorder management and MAT by coordinating a robust internal network of Addiction Medicine specialists, Behavior Health professionals, Buprenorphine-waivered medical practitioners, internal and external OTPs, and other community partners.
       

    • YesCare has created training, via Lean Six Sigma, to increase proficiency with withdrawal protocols and MAT Maintenance/Induction processes. Training includes healthcare professionals as well as our correctional partners. Patient education and reentry services provide our patients with the tools and resources that are essential in managing their addiction.

Telehealth

  • Comprehensive continuum of care: We are committed to providing the right level of care for each patient based on their unique needs and circumstances. To do this, we provide a full continuum of behavioral health services, from short-term outpatient care, to inpatient psychiatric hospitalization. Within each setting, we tailor programming to support recovery and, when ready, transition the patient to another level of care where we can continue to reinforce recovery while building autonomy through evidence-based intervention.
     

  • Telepsychiatry: Telepsychiatry is a proven method to increase psychiatric access for patients at single facilities and across entire systems. It is as effective as in-person care. Telepsychiatry enables organizations to access psychiatric coverage without the recruiting, logistical and financial burdens that the onsite, in person model requires. Telepsychiatry increases provider efficiency, adds value and improves care.

  • Tele-behavioral health (e.g., tele-therapy): Like telepsychiatry, tele-therapy is evidence-based and remains essential to increasing access to care in the community. And it can do the same in correctional institutions. From BH screening and assessment, to psychotherapy and crisis evaluations, tele-behavioral health increases access, reduces escorting needs and consequently reduces risk and improves outcomes.

  • Suicide prevention: YesCare introduced our Suicide Reduction Task Force in 2019. In our first year, we realized a 10% reduction in suicides across our network of care, despite a significant rise in jails and prisons nationally. We are committed to reducing the number of suicides to zero and are building best-in-class tools and programs to support our professionals in the field and are establishing partnerships with cutting-edge companies to address suicide risk with novel, yet proven, interventions.

  • ​Psychiatric medication management: YesCare ensures evidence based prescribing by measuring prescriptive practice and reducing polypharmacy.  Our proven tools reduce cost for our clients and promote patient safety.  We teach, coach and train providers individually and in group settings to ensure that patients receive evidence based medication trials that match their DSM-5 diagnoses.

Infectious Disease Management

  • Per CDC, approx. 1.2 million people in the United States have HIV (13% undiagnosed) with the following: 

    • 65.9% received some HIV care 

    • 50.1% were retained in care 

    • 56.8% were virally suppressed or undetectable.

 

  • HIV diagnoses are not evenly distributed.  

    • In 2019, the rates were the following: 

      • 15.2 in the South 

      • 9.4 in the Northeast 

      • 9.2 in the West 

      • 7.2 in the Midwest.  

  • New HIV infections fell 8% from 2015-2019: 

    • ÝesCare has played an essential role in the decline of HIV infections with screening, testing, patient education, counseling, medication selection, follow-up care and community resources making up the foundation of our comprehensive, multidisciplinary approach to HIV care. For new and/or established cases, our YesCare team understands the importance of establishing care and continuing medication management to optimize wellness of our patients.   

    • YesCare has vast experience with cost-effective care for HIV treatment including use of the Ryan White Foundation, 340b pricing, partnerships with local Departments of Health and Academic Medical Centers, such as University of Kentucky Infections Disease Clinics who helps with medications at release.
